Acute & Repeat Prescription Management
Acute and repeat prescription management helps to reduce the workload on busy clinicians by using our clinical pharmacy team to respond to the day to day prescription request demands. Our proactive repeat prescribing process helps reduce the volume of these requests over time as well as supporting practices to identify patients overdue clinical review.
Medicines Reconciliation
Medicines reconciliation is the process of identifying an accurate list of a patient’s current medicines (including over-the-counter and complementary medicines) and carrying out a comparison of these with the current list in use, recognising any discrepancies, and documenting any changes.
Targeted & Structured Medication Reviews
Targeted medication reviews focus on optimising patient outcomes around specific target medicines. Examples of targeted medication reviews including opiate prescribing reviews, benzodiazepine reduction reviews and review of medicines likely to increase anticholinergic burden. Structured Medicine Reviews (SMRs) are an evidence-based and comprehensive review of a patient’s medication, taking into consideration all aspects of their health.
Medicines Safety Audits

Medicines safety audits constitute either periodic or regular audit of target medicines. These audits usually relate to medicines associated with higher patient risks e.g. Disease-modifying anti-rheumatic drugs, shared care drugs or drugs with narrow therapeutic indexes. Regular audit of these medication groups can help practice compliance with CQC requirements.
QOF / Therapeutic Reviews
We are currently supporting our primary care clients with hypertension reviews and lipid optimisation reviews. Further support for other long-term conditions is presently under development.
